Attention: upcoming 7th and 9th graders only!! According to the TN State Board of Education rules you are now required to have a "Well Child Check-up" or and EPSDT physical examination. This must be completed by your family Medical Doctor. This download will provide you with the TSSAA physical forms and the SBE form. Both forms MUST be filled out four your sports participation. 9th Graders Please return these forms to your High Schools Athletic Trainer. 7th graders must return the SBE form to your school nurse and your TSSAA form to your coach. The Tennessee Secondary School Athletic Association (TSSAA) requires a screening of all high-school and middle-school athletes before they try out or practice for school sports. The Fort Sanders Sevier Therapy Center is proud to have hosted the screenings for local students for more than 15 years. The Fort Sanders Sevier Therapy Center in Sevierville is located behind Fort Sanders Sevier Medical Center at 709 Middle Creek Rd.
